Student Registration: New Students



Dear Parent or Guardian:

On behalf of the Greendale School District, we would like to welcome you and your family to our schools. We hope that your child's experience will be most pleasant and productive.

In order to facilitate the enrollment process, please bring the following required items with you to register your child:
  • PROOF OF RESIDENCY – The district requires two original documents. Please present proof of residency when you visit the school to complete the registration process. At least one document must be a mortgage statement, a signed apartment lease, a property tax statement or closing statement for purchasing a home. The second document can be a utility bill, driver’s license, auto or health insurance statement, Medicaid/Badger Care benefit statement or pay stub.
  • IMMUNIZATION INFORMATION – Please bring your child’s immunization information when you come to the school for registration so you can complete the Student Immunization Record. This record provides written evidence of immunization against certain diseases as required by Wisconsin law. Students transferring from other public or private schools within Wisconsin are given 30 school days to complete and return the immunization record. After 30 days, the students must be excluded from school if the record is not received. No student transferring from out of state may attend school until the immunization record is completed.
  • BIRTH CERTIFICATE – Please bring an original or copy of your child’s birth certificate or passport.
Greendale Middle School’s office hours are Monday – Friday, 7:00 AM – 3:00 PM.
