Parent Signature Forms
Planning a vacation that will impede on several school days? If your child must miss school, it is recommended that the parent contact the school office to obtain a Extended Student Absence Form in advance of the absence so that the teachers can best prepare your child for time out of school. Some work may be given to take on the trip, but most often teachers will work with your child after their return to “catch up.” Students in this situation should plan on spending some extra time with their teachers upon their return so that the student transitions well back into the academic program.

Student Drop-Offs
Students often forget things at home – instruments, lunch money, agendas, homework – and parents sometimes bring these items in to school for them. To minimize distractions for both the classrooms and in the school office, these items will be held for pick-up at either lun ring our HomeBase period at the end of the day. Students are responsible for checking for their items at those times.
When do students need to be at school?
There is no student supervision provided prior to 7:44 a.m. Students enter the building at 7:44 a.m. and are dismissed at 3 p.m. When students arrive in the morning, they are to go to their lockers and then to their first hour class that begins at 7:54 a.m.
What are the procedures for reporting an absence or tardy?
Parents must call school (414) 423-2800 each day of the absence by 9 a.m. If no call is received, our attendance secretary will call to inquire about the absence. All absences that are not approved by a parent will be considered truancy.
If I need to schedule an appointment that will cause my child to leave school for part of the day, are there good times to do this?
Obviously, the school recommends scheduling appointments outside of the school day or on vacation days if possible. If there is a need to take a child out of school, we recommend that it is scheduled over the student’s lunch period or at the end of the day.
We have HomeBase each day from 2:50 to 3 p.m. and this would be a good time to excuse a student from school.
What are the procedures if my child must take medication while at school?
If it is necessary for a student to take medication at school, be it over-the-counter or prescription, the following steps must be taken: A note from the physician and the parent must be turned in to the school office immediately. The medication must be brought to the school office labeled with the student's name, dosage, all instructions, and the medication must be in its proper container. Students may carry their inhaler with them, as long as a medication form is on file in the office. For more information, please click here.
When is there a secretary available to take my calls? How can I reach staff?
The school office is open from 7 a.m. until 3:45 p.m. Teachers are available from 7:15 until 7:55 a.m. and from 3 p.m. until 3:15 p.m. All staff can also be reached through the voice mail system or through e-mail.
How do I get in touch with the school outside of these hours?
We do not have consistent staff to answer the phone outside of these hours. Access the middle school’s voice message center by calling (414) 423-2800.
If the weather or another situation warrants a school closing, how will I know?
In the event that school is closed because of severe weather, you will be informed through the local media. TV stations 4, 6, and 12, as well as radio stations WTMJ, WKTI, WEMP, WISN, and WOKY will have accurate information. When school is closed because of bad weather, after school and evening events are automatically canceled.

The clinic is generally open Mondays from noon until 4:00 p.m. and Fridays from 7-11:00 a.m. The program is coordinated in partnership with Aurora Health Care.

Authorization for School Personnel to Administer Medication to Student
Any prescription medication that will be administered at school must be accompanied by a district prescription medication form signed by the parent and physician. If a student will carry an inhaler, the same form is required with the parent and physician's signatures. A parent signature is required on the form for any non-prescription medication. Special Education
The Greendale School District offers a continuum of services for children who qualify for special education and related services, aged 3-21. All children suspected of having a disability are referred for an evaluation to determine if they have a special need.

The Greendale School District offers a continuum of services for children who qualify for special education and related services, aged 3-21. All children suspected of having a disability are referred for an evaluation to determine if they have a special need in one or more of the following areas:
- Autism
- Cognitive Disability
- Emotional/Behavioral Disability
- Hearing Impaired
- Speech/Language Disability
- Visually Impaired
- Traumatic Brain Injury
- Specific Learning Disability
- Orthopedically Impaired
- Other Health Impaired
- Significant Developmental Delay
If a child is determined to have a special education need, an Individualized Education Program (IEP) is developed to identify the necessary goals, instruction, support and related services so that the child may benefit from the educational process. Parents are an integral part of the evaluation and the development of the IEP.

Opportunities exist in the classrooms working directly with teachers and students, assisting in the Health Room, making copies, chaperoning field trips, supervising at recess or at lunch time, or helping out at a PTO function or extracurricular event. Volunteering can be very rewarding – which is why many of our volunteers return year after year.
