In order to facilitate the enrollment process, please bring the following required items with you to register your child:
- PROOF OF RESIDENCY – Please present proof of residency when you visit the school to complete the registration process. Acceptable proof of residency may be a copy of an Offer to Purchase a home within the Village of Greendale, a copy of an apartment lease, utility bill, or other acceptable form of residency.
- IMMUNIZATION INFORMATION – Please bring your child’s immunization information when you come to the school for registration so you can complete the Student Immunization Record. This record provides written evidence of immunization against certain diseases as required by Wisconsin law. Students transferring from other public or private schools within Wisconsin are given 30 school days to complete and return the immunization record. After 30 days, the students must be excluded from school if the record is not received. No student transferring from out of state may attend school until the immunization record is completed.
- BIRTH CERTIFICATE -- For children enrolling in kindergarten or private kindergarten students entering first grade, please present a birth certificate establishing the child's age. (This does not apply to transfer students in grades 2-12.)
