2016-17 Wellness Program Requirements
To be eligible for the 2017 HRA reimbursement ($1,750 single/$3,500 family), you must complete the following items by December 31, 2016. Employees who do not complete the wellness requirements will not be eligible for any deducible reimbursement from the District.
Register and complete a Health Assessment on the HumanaVitality site. Login to create your account and get started!
Register for and complete an onsite Biometric Screening. Biometric screenings are being held onsite on the following dates:

Spouses/domestic partners on the GSD insurance plan will also need to complete a biometric screening. This can be done onsite at one of the school locations*, at a QuickCare clinic, with Sharon Solomon (GSD onsite Nurse Practitioner), through their employer’s onsite screening, or with their own provider.
*Please note: Spouses can use the same registration information detailed above to sign up for an onsite screening. The can register and come for any of the onsite times (they just need to check in through the school office) and we are also offering a Saturday session on October 15th at the High School for their convenience. If your spouse comes to the onsite screening or schedules to complete their screening with Sharon Solomon, their results will be automatically uploaded to the Vitality site.
If your spouse completes the screening elsewhere, he/she will need to use this form to enter the results from the biometric screening on the Vitality wellness site. Humana will provide the district with a list of employees and spouses that have completed the biometric screening (without any results attached; it is just a confirmation that it has been completed).
